Tamara Neely, is a family nurse practitioner with extensive training as an HIV specialist from Duke University. She completed her 2nd post Masters certificate as a psychiatric mental health Nurse Practitioner at Anderson University in Anderson, SC in May 2019. She graduated from UNCC as a Family Nurse Practitioner in 2014. She has a speciality niche business, HomeCare for the Carolinas LLC (HC4C) which is a Historically Underutilized Business (HUB) and Minority, Women Small Business Enterprise (MWSBE). HC4C works extensively with the government (HRSA, HUD & VA). Since 2006, HC4C has been a successful business with a team of 20.
HC4C provides the following services:

Non-medical home care services,
HIV Case Management, contract pharmacy,
and two VA contracts for transitional group homes for homeless male veterans in the Charlotte metro area with mental heath disorders and/or substance abuse issues.

Tamara is the founder of The RISE Project of the Carolinas, a 501(c) 3 that works with individuals with mental health disorders providing VA transitional housing to male veterans in the “Hospital to Housing Model” and HIV Case Management with a focus on the minority population and has been helping those since 2008. The annual World AIDS Day Program (Dec.1) has been a staple program in the community to educate others about HIV and bring awareness and prevention.
Her real estate company, Neely Realty & Investments, is a real estate broker firm in Charlotte, NC with residential and commercial rental properties with eleven doors.

HC4C Clinic opened August 2020 as a mental health clinic to assist those with substance abuse problems and mental health issues. In addition, HC4C Clinic offers massages and facials and weight loss options in addition to helping those with HIV and those with opioid addictions by prescribing suboxone. Mind, body, and soul will be our focus.
